*Please note that if you are registering on behalf of someone and you would like to purchase a WSPHA membership with their conference registration, you will need to complete the form as if you were the individual you're registering for, and either log into the person's account or create a new account on their behalf. Cancellation Policy
The Washington State Public Health Association incurs costs based on the number of expected attendees, and late cancellations impact our ability to continue to offer a high-quality conference and learning experience for our participants. WSPHA can offer refunds based on the schedule below.

Request received by:
July 3rd: 100% refund
August 4th: 75% refund
September 1st: 50% refund
Between September 2nd and 15th: 25% refund
After September 16th: No refund
